Creating a new Form
WEB APP
To create a new
Form
template, navigate to the
Admin Area
in your D4H
Incident Management
account.
Open
Templates
>
Forms
Click on
New form template
Fill in the details and create your template using the
Form Builder
​
Click
Save
at the bottom of the screen
The name will be the name of the form that appears in the Forms module and will be the header on the PDF printout of the form.
Names should be unique and clear. For example, EMT Meeting Agenda, ICS 208 Safety Message, Hazardous Materials Report, etc.
